DESCRIPTION THERAPIST Statements of Purpose Under the supervision
of the Clinical Director the Therapist is responsible for
completing the Biopsychosocial assessment, developing treatment
plans, conducting process groups and individual sessions with
clients. Major Task, Duties and Responsibilities Oversee the daily
treatment experience of each of his/her clients. Maintain
appropriate documentation as required. Conduct assessments of
clients at the time of admission and re-assessments as required.
Conduct mental status examinations. Provide appropriate diagnoses
and treatment recommendations based on a review of the clients
history, signs, and symptoms. Provide individual, couple/family,
and group psychotherapy as needed. Ability to demonstrate respect
for cultural and lifestyle diversities of clients and staff. Crisis
intervention services, including 5150 determinations. Treatment
Planning. Participation in Case Conference Reviews. Complete
Utilizations Review forms as required. Knowledge of current legal
standards pertaining to professional practice, reporting
requirements, professional boundaries, and related issues.
Knowledge of the philosophy and approach of the 12-step model of
recovery (AA, CA,NA,Alanon). Charting Requirements Page 1 of 3
Initial Treatment Plan must be complete and signed within 24 hours
of admit. Treatment Plan Updates must be completed every 7 days.
Biopsychosocial must be complete and signed within 72 hours of
admit. Clients must receive at least one individual sessions with
primary therapist each week. Notes must be logged same, day,
including notes that state if and why client was not seen, etc. If
clinician leads groups, notes must be logged same day. Referents o
Must contact referent within 48 hours of admit. Call must be
documented in client chart. o Must notify referent within 72 hours
of clients treatment plan. Call must be documented. o Must contact
referent minimum of 2 times per week via phone to keep family
and/or referent updated, including weekly update of client
treatment plan. Call must be documented.
